密碼登入請求配額；成功與失敗的認證請求都計數，與 LoginLockout 的失敗鎖定分開。

HTTP adapter 在密碼驗證之前呼叫，clientIp 必須取自實際連線或可信代理接線； username 的正規化依實作契約。分散式配額可提供另一實作，不可將普通 cache get/put 視為跨節點的原子操作。
